EyeMed Vision plan offers a vision-care network that includes major vision care providers such as LensCrafters, Pearle Vision, Target, and Macy's as well as a network of private practitioners. Using an in-network provider allows you to receive care at no cost or minimal out-of-pocket expense.

Finding a Provider

To find a list of EyeMed providers, click here and follow the directions below:

• Click on the drop down menu under "Locate a Provider" on the EyeMed website
• Choose "Select" and enter your zip code
• Click the "Submit" button to see a list of providers in your area
